Clarion Forest VNA - Healthcare In Your Home
- Two reports recently released by Lewis Research, utilizing data from the Center for Medicare Services (CMS) showed very postive results for Clarion Forest VNA. The "Outcomes Dashboard" compares the outcomes of our patient care with other home health providers in our local area, in our state, and in the nation. We received a "Great Score" of 92% with 11 of the 12 outcomes that are measured, being above the local, state and national averages. The "Percentiles Dashboard" showed us to be at the 80th percentile in the state overall ranking and the 84th percentile nationally (no local market percentile is done). We are extremely proud of our results and continuously strive for improvement.
Clarion Forest VNA has been named to the ‘2009 HomeCare Elite’, as one of the top performing Home Health Agencies in the country. This compilation is an annual review which identifies the top 25 percent of agencies ranked by an analysis of performance in quality outcomes, quality improvements and financial performance. The review is brought to the industry by OCS Homecare, the leading provider of home care information, and DecisionHealth, publisher of homecare’s most respected independent newsletter – Home Health Line.
